Case Management

Friend Family Health Clinic offers case management to pregnant women. A case manager is able to enroll eligible pregnant women for Medicaid coverage. This coverage, provided in collaboration with the Department of Human Services, enables these women to receive prenatal care during their pregnancies.

FFHC has an extensive social services department that includes a licensed clinical social worker, family case managers, legal services, and a Help Desk, with volunteers ready to assist you in getting through the social service system.

Case management services include:

• Assessment and referral
• Coordination of medical care
• Empowerment of participants
• Home visitation as required
• Periodic follow up
